Finding your Membership Number
Your UCare Membership Number can be found on your UCare Membership Card that comes together with the UCare Kit that you purchased.
Finding your Product Serial Number
- Your Product serial number is usually stated on the Product purchase invoice/receipt.
- You can also find your Product serial number on the sticker attached to your Product:
- For Notebook: The sticker shall be underneath your Product (but NOT on battery)
- For PC: The sticker shall be on the back panel of your Product
- For Projector: The sticker shall be underneath your Product
Fax your completed application form to 1-800-888-FAX (329) along with a copy of the proof of purchase of your IT Product.
You will receive a confirmation email with the details of your UCare Membership. You are advised to save or print a copy for your own record.
